This is a toughie.
Anything could be happening. It is possible that medication is not
working. It is probable that the ADHD is interacting with the usual
difficulties of 15 year olds.
It seems the biggest problem is that there are few, if any, people
around you who understand this condition.
Can you find a local support group? You mentioned therapy, but I know
from my own experience that some therapists don't have a clue. People
with ADHD need to get in touch with themselves, assess their unique
strengths and weaknesses, learn how to do things in ways that work for
them. Too many therapists try to make us "normal." It is like shaving
down a square peg to make it fit a round hole instead of trying to find
the right hole. A support group can help. Professionals with close
family members who are ADHD and are well-adjusted can help, too.
Finally, has your son had a full medical check up? Sometimes a ADHD can
cause medical people to exclude other causes. A couple of years ago
they put me on a doubled dose of Ritilan. It didn't seem to help, so
the doctor wanted me to take a higher dose. I went to another doctor.
As it turned out, I had an undiagnosed persistent sinus infection.
